Full Time RN Home Health Case Manager - Registered Nurse (RN) As a Home Health Case Manager - Registered Nurse (RN) you will be responsible for providing nursing care to clients and direction and leadership to designated staff. Additional responsibilities of the Home Health Case Manager include: Assessing and formulating a plan of care for clients in their homes that includes social, emotional, physical, and spiritual needs.
