About Us

ColumbiaCare Services is a non-profit, mental health treatment agency offering a full spectrum of behavioral health care. We are more than a company. We are a team of people who are in the business of changing people's lives. We specialize in providing outpatient services, residential treatment programs, mental health housing, and affiliated services for individuals living with mental illness. We are also proud to serve Veterans.

About the Program

Hourglass Community Crisis Center is a program designed to provide 24/7 support to individuals who suffer from mental illness. This Crisis Center will be available 24/7 to accept individuals referred by law enforcement, community providers and individuals themselves for mental health crisis assessment, stabilization, and active linkages to community providers for same-day or next-day follow-up support services.

About the Position

We have an opening for a Follow-Up Specialist position at our Hourglass Community Crisis Center in Eugene, OR. ColumbiaCare Services is looking for the right individual to work peer to peer, (meaning you are affected by mental illness), and are willing to share your personal experiences with adult consumers affected by a severe and persistent mental illness, while effectively managing your own symptoms.

Work Schedule:

This position will assist Trillium CCO Members who present at Hourglass Community Crisis Center to set up appointments with medical providers and other community treatment/ resource providers. Arrange or provide transportation to these next-step appointments. Provide “Virtual Escort” services (call ahead of time to let the receiving agency know the person is coming, and follow up after the appointment with a text, call, or email to determine if the person arrived and was welcomed at the receiving agency). Remain in contact with people who have received services at Hourglass to maintain motivation and limit frustration until the person is connected to services. Follow up with the Member to assess Member satisfaction and connection to on-going services. Record work in the electronic clinical record. In addition, while we are an organization that does not practice seclusion or restraint, we do still require our direct care staff to successfully complete our Verbal and Physical Intervention (VPI) crisis de-escalation training, which includes performing various physical, evasion and safety maneuvers in class.
