Schedule: Mon - Fri, 8a to 5p
The Case Manager provides recovery services to adults with severe and persistent mental illness. Follows evidence-based practices using the Strengths-Based model of case management, Assertive Community Treatment (ACT), and other program-specific evidence-based practices in a multi-disciplinary team.
Responsibilities
Client services are primarily delivered in the community (home, place of employment, neighborhood) rather than in an office setting. Case Managers help carry out individual treatment plans, which may include providing assistance with entitlements, employment, housing, accessing health care, etc. Facilitates psychoeducational/skills training groups. Regularly drives agency vehicle and transports clients as required. Submits timely documentation of services provided using Electronic Health Records and collaborative documentation.
Qualifications
Candidate must possess a Bachelors degree in a human service field plus 2 years experience with similar population preferred, OR an equivalent combination of education and experience. CADC a plus.
Candidate must be able to work independently and flexibly in a multi-disciplinary team. Experience with substance abuse treatment with an understanding of co-occurring disorders preferred. Computer proficiency required to use Electronic Health Records. Must pass state-required background and DMV checks, possess a valid OR driver's license, and demonstrate an excellent driving record to transport clients.
