Family Practice Clinician sought for highly ranked integrated adult primary care clinic with on-site behavioral health providers, dental services, pharmacy services and administrative support. The provider delivers comprehensive primary care services as part of an adult primary care clinic which is located within our adult outpatient mental health facility.
Responsibilities
The provider diagnoses, treats and helps prevent acute and chronic illness. Provides physical examinations, orders and interprets lab tests, and provides acute and chronic care services to clinic patients, in accordance with Federal, State and clinic statutes and protocols. Reviews, evaluates and collaborates with psychiatric and behavioral health providers regarding treatment plans, client progress and outcomes. Responsible for ensuring compliance with coordinated care organizations' patient health matrix.
Qualifications
Completion of an accredited nursing program required and ability to obtain Oregon Nurse Practitioner license.
Must maintain an active, unrestricted Oregon professional license with prescriptive privileges and current DEA registration.
Computer proficiency required to use Electronic Health Record system. Must pass criminal history and DMV checks; valid OR driver's license with acceptable driving record required.
