The Peer Wellness Coach works as an active member of the treatment team helping to reach out and engage people into needed mental health, addictions and physical health services. The Peer Wellness Coach provides support, education and mentoring to program participants to assist them to develop personal goals toward health and wellness and to envision and achieve a meaningful life.

Responsibilities

Utilizes excellent communication and interpersonal skills to make the process of learning about services and accessing services a positive experience for each individual. Performs outreach to individuals who may benefit from participating in Wellness Coaching. Co-facilitates recovery and health related education groups. Assists individuals in problem solving around personal issues impacting on their ability to access services (i.e. transportation, remembering appointments). Assists individuals seeking services with completing required paperwork. Assists with gathering basic health information, provides education on services and gathers other relevant information from individuals. Assists Options' clients in using community resources to enhance their independent living skills. Informs lead staff persons, nurse, clinicians, program managers and other pertinent parties of clients' needs or concerns. Performs outreach both via phone and in person in the community to promote engagement in services. Transports clients via agency vehicles to promote engagement in services and access community resources.

Qualifications

High school diploma or equivalent and a general understanding of the population served. Some post-secondary coursework preferred. Bachelor's degree in a behavioral sciences field; or a combination of at least three years relevant work, education, training or experience a plus.

Have a history of receiving mental health services and be well-grounded in one's own recovery, with at least a year since diagnosis preferred. Knowledge and skills sufficient for establishing effective relationships with clients who have a mental illness in order to support them in their own recovery. Ability to follow verbal and written instructions; ability to work independently and with staff as agency needs dictate. Must be able to complete program-specific training and Peer Certification (provided by Options).

Candidates must be able to use e-mail, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system. Must pass State-required background check and DMV checks; possess a valid OR driver's license and demonstrate an excellent driving record.
