PAY AND BENEFITS

Annual Pay Range: $48,365.75 - $60,943.64

Hourly PayRange: $24.802950 - $31.253151

Starting salaries will be within the posted pay range, based on job-related factors, such as relevant experience. This may include military service, paid work, and unpaid work (internships, volunteering, etc.). In addition to competitive wages, Clackamas County offers an attractive benefits package for employees in regular status positions.

JOB DETAILS AND QUALIFICATIONS

Clackamas County Social Services Division is looking for a Case Manager with an outreach focus in order to reach Veterans, determine eligibility, conduct comprehensive ongoing assessments to clients, and provide follow-up case management as well as provide backup as needed to other Case Managers who work with Veterans in the County. The Case Manager in this position will use a strengths based, person centered approach to provide outreach, engagement and supportive service for veterans and their families that are experiencing homelessness or at risk of homelessness. This includes eligibility determination, obtaining & retaining safe and affordable housing in Clackamas County and partnering with a wide variety of community resources. All services will be delivered in a manner that is culturally responsible including military culture.

Successful candidates will have a strong working knowledge of laws, rules, and regulations applicable to veteran's benefits with a background in working with Veterans and the ability to assess client needs and apply them to appropriate programs, services, and entitlements. The ideal candidate will be a trained veteran with a strong background and experience in providing assistance to veterans experiencing homelessness or an imminent housing crisis.Required Minimum Qualifications/ Transferrable Skills:

A minimum of two (2) years of relevant experience in case management or a comparable function with veterans and/or civilians experiencing homelessness.

Any combination of relevant experience (paid or unpaid) and education that would likely provide the required knowledge and skills, may substitute for the required years of experience.

At least one (1) year using creative problem solving, with successful results, in preventing people at high risk of homelessness from becoming homeless or helping people who have been homeless access and maintain permanent housing.

In-depth knowledge, demonstrated by veteran status, education, training and/or working experience, of veteran needs and advocacy.

Excellent interpersonal communication skills.

Experience building relationships and networks with local landlords, program participants, colleagues and community partners.

Demonstrated commitment and experience advancing equity and diversity in a workplace or community setting.

Must pass a post-offer, pre-employment drug test. Learn more about the County's drug testing policy.

Must pass a criminal history check which may include national or state fingerprint records check

Driving is required for County business on a regular basis or to accomplish work. Incumbents must possess a valid driver's license, and possess and maintain an acceptable driving record throughout the course of employment. Learn more about the County's driving policy

Preferred Special Qualifications/ Transferrable Skills:

Veterans or those with military service are strongly encouraged to apply.

Knowledge of Clackamas County and regional resources available to help homeless veterans.

Familiarity with Service Point Homeless Management Information System.

Experience using State of Oregon Emergency Housing Account funds.

Trained in Assertive Engagement.

TYPICAL TASKS

Duties may include but are not limited to the following:

Use a variety of outreach methods to locate veterans who are homeless in Clackamas County. Outreach to include visits to homeless camps, meal sites, temporary shelters and winter shelters as well as other sites as appropriate. Use a nonjudgmental, strengths based approach to build professional relationships with identified homeless veterans.

Assess each veteran experiencing homelessness or at imminent risk of homelessness who requests help with housing using the Coordinated Housing Access system.

Accept referrals from Coordinated Housing Access system. Gather documentation and information to confirm initial program eligibility and re-certify annually if required by funding source.

In partnership with homeless and formerly homeless veteran participants, use a strengths-based, non-judgmental approach to develop and implement financial and service plans to meet housing, income, health, educational and other needs of participants; determine appropriate treatment, training and care services; match participant needs with potential providers.

Participate in Homeless Veteran Coordination Team, compile and distribute Veterans By Name List to documented partner providers, represent Social Services Division at Neighborhood Livability Team and other networks as requested.

Coordinate with health care providers, social service personnel, veteran's administration and other agencies to obtain services; reevaluate and update case plans and client eligibility on a regular basis; advocate for client with providers, public and private agencies, law enforcement agencies, justice system, landlords, schools and others.

Prevent emergencies whenever possible through pro-active communication, including with landlords and property managers. Respond to emergencies and negotiate resolution of problems. Report witnessed or suspected abuse and neglect of children, older adults and/or people with disabilities.

Identify local resources available to address unmet needs of veterans and others who are homeless, formerly homeless or at high risk of returning to homelessness. Provide information and referral services to clients and family members. Build relationships with community partners to better understand services available and to share information about CCSSD using opportunities for networking and continuing education.

Document services provided to participants. Provide and document follow-up contacts with participants at intervals required by funders. Submit necessary documentation to implement security deposit, rent and flexible participant assistance payments.

REQUIRED KNOWLEDGE AND SKILLS

Working Knowledge of:Methods and techniques of case management; social, health care, financial and other community resources and agencies; principles and techniques of interviewing, counseling and crisis intervention; laws, rules and regulations applicable to government programs and funding guidelines, including Title XIX; principles and techniques human development and behavior in assigned area, such as geriatrics, physical disabilities, developmental disabilities, chronic mental illness, drug and alcohol abuse, or economically disadvantaged; office equipment, including personal computers and software programs.

Some Knowledge of:Policies and procedures related to protective services, including client rights, hearings procedures and legal terminology; laws, rules and regulations applicable to involuntary commitment, protective services, guardianship, conservatorship, informed medical consent, confidentiality, and facility certification and regulation; medical criteria for levels of care, disease processes and medical terminology.

Skill to:Gather information through research and interviews with applicants, clients, families, care providers and others; apply comprehensive social service evaluation and assessment methods; develop and implement appropriate case plans for clients with diverse needs and diverse settings; identify community/client needs and develop programs and resources to meet those needs; interpret and apply federal, state and local laws, rules, regulations and policies; analyze relevant data and authorize expenditure of public funds in compliance with guidelines; compute payments for clients/program related expenses; maintain accurate and concise records; communicate effectively, both orally and in writing; establish and maintain effective working relationships with clients, families, care providers, community resources, other public and private agencies and County employees; operate computer software and other office equipment.

WORK SCHEDULE

This position is included in the County’s alternate workweek program, working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
