Position Title: Registered Nurse (RN)Salary:                ÂDOE, Full BenefitsStatus/Hours:           Non-exempt, 40 hours per week Reports to: Nurse Manager ABOUT THIS POSITION Our registered nurses work with new and existing patients in the development, implementation, and evaluation of individualized care plans for high risk, complex patients. They advocate for the safety of patients and serve as a liaison between healthcare providers, community services, families, and the patients themselves. We believe healthcare extends beyond the exam room, and focus on serving the whole person with support and resources that engage them fully in creating good health. ABOUT EMPLOYEE BENEFITSAll full time employees are eligible to participate in Wallace's benefits offerings, which include health insurance through Kaiser Permanente (including dental and vision), generous paid time off, 9.5 paid holidays per year, a 3 - 4% matched 401(k) retirement plan, flexible spending accounts for medical and/or dependent care, and fully paid basic life and disability insurance.ÂJOIN THE WALLACE TEAM The WMC staff is our greatest asset in providing essential, high-quality health services to the greater Portland area. And as the community we support grows, our team grows with it. We offer an exceptional workplace the values individual contributions, cultural diversity, and ongoing learning. When you join our team, you will… …Bring good health to overlooked and underserved individuals and families in our community …Collaborate with dedicated staff and volunteers in an open, cooperative, and respectful environment JOB DUTIES AND RESPONSIBILITIES: Clinical areas of responsibility Work closely with providers to enhance patient experience, understanding and involvement in their own health care Phone and walk-in triage and advice for all types of patients per licensing, standing orders and protocols, providing immediate response when needed for urgent triage Prescription refills and medication adjustments per standing orders and in collaboration with providers Review daily referrals, meet with clients to review treatment plans, follow-ups and psychosocial needs Assist clients in establishing individualized health goals, identifying potential barriers, providing education resources and advocacy, and monitor patient progress and health goal adherence Assist in timely scheduling of primary care and other appointments to support client health Review medication lists, changes to medication regimen and medication use Clarify medical terminology and assist patient in understanding current health status and subsequent goal setting tools that will be used to encourage patient improvement Link individuals to application assistance for health coverage programs Maintain good working relationships with community health partners by working as a liaison with other agencies and the community Coordinate with grant and program managers to develop innovative programs Wallace Team Member Nurture healthy working relationships with staff, volunteers, and patients Maintain strict patient, staff and organizational confidentiality Work as a member of the clinic team Demonstrate a deep concern for the health and well being of others Participate in compliance, quality assurance and quality improvement activities as directed Perform other duties as assigned REQUIRED QUALIFICATIONS, SKILLS, AND ABILITIES Required: Successful completion of an accredited Registered Nurse program Current Oregon RN license At least an Associate’s degree in Nursing At least 1 year of experience in a clinic setting as a nurse, or 2 years as a medical assistant Current CPR certification Working knowledge of state healthcare laws, licensing board regulations, and controlled substance regulations Ability to work independently and in a multidisciplinary, team-oriented environment Preferred: Bachelor's degree in nursing preferred Experience working in a community health clinic preferred Knowledge of data and chronic disease registries a plus Bilingual in Spanish preferred; all multilingual speakers highly encouraged to apply WORKING CONDITIONS Duties are performed primarily in an office and clinic environment. Some evening or weekend hours may be required. PHYSICAL REQUIREMENTS Ability to work in a fast-paced environment.
