We are seeking a Health Services Coordinator to help fulfill our passion for providing the best care to our residents.

About the Position: Our Health Services Coordinator is responsible for nursing assessments, wound care, working with physicians and other medical and mental health providers, coordinating home health care when necessary, overseeing caregivers and medication aides, working with administration and all the care team members and assessing new residents that are looking to call Prairie House home.

This is a full time position, working Monday through Friday, with some flexibility for evenings or weekends, based on our residents’ needs.

Qualifications: Current unencumbered RN license with the state of Oregon. Experience working with seniors and demonstrated knowledge and experience with residents having dementia and the understanding of the physical, mental, and social changes associated with the elderly population. Demonstrated ability to organize, problem-solve, and obtain results in seniors. Must be able to successfully pass a criminal background check.

We Offer a Benefits Package That Includes: Employees that work 30 hours or more a week are eligible for our comprehensive benefit package that includes medical, dental and vision. Other benefits include 401(k) retirement savings. Vacation and sick accrual. In addition, we are proud to offer access to our educational grant for employees that have served one year of employment.

Student Loan Repayment Program: This benefit allows licensed staff who have completed 120 days of service and average 24 hours (worked hours) a week, to apply for assistance in paying their student loan debt. Up to $200 per month!

Sign-on and Retention Bonus: We are offering a $1,000 sign-on bonus, paid the first payday following date of hire. We are also offering $1,750.00 paid after six months (must be in good standing and have no disciplinary actions) and an additional $1,750.00 paid after one year (must be in good standing and have no disciplinary actions).
