DescriptionThe Behavioral Health Care Manager, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness by guiding members/families toward resources appropriate for the care and well being of members. The Behavioral Health Care Manager's work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.ResponsibilitiesThe Behavioral Health Care Manager employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.This position will support Humana members' access to behavioral health services. The role provides member outreach and education related to behavioral health, including assessing risk, determining appropriate care in individual patient communities, and collaborating with providers as well as clinicians across other Humana departments.This position will work an 8 hour shift-Monday-Friday.The shift includes hours between 9AM and 8PM Eastern Time with occasional overtime offered.Required QualificationsCurrent valid social service license, e.g. MSW, LCSW, LMSW, LPC, LMHC

Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university

1-3 years or more of experience working as a behavioral health professional

Remote RequirementsRemote requirements: Must have the ability to provide a high speed DSL or cable modem for a home office. Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.

A minimum standard speed for optimal performance of 25x10 (25mpbs download x 10mpbs upload) is required.

Satellite and Wireless Internet service is NOT allowed for this role.

A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information

Preferred QualificationsExperience supporting patients telephonically

Proficiency with Microsoft Office Programs, e.g. Word, Excel & PowerPoint

Experience with behavioral change, health promotion, coaching and/or wellness

Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team

Experience working with both children and adults

Bilingual (English/Spanish)
