Summary of Responsibilities:

Under the general direction of the IDD Unit Supervisor, this person provides support coordination services for consumers. Services are provided in accordance with all existing policies, procedures and regulations and in accord with the mission and values of the company.

Essential Functions:

 Completes required documents to request authorizations for services

 Assist in identifying and accessing community resources & providers

 Verify MA status/insurance eligibility

 Complete referral to appropriate agencies and/or units within SAM

 Provide crisis intervention

 Provide support to consumers

 Participate in interagency planning

 Attend court hearings

 Complete written reports

 Maintain accurate and up-to-date files

 Complete service notes

 Complete release of information processes

 Participate in discharge/transition planning

 Complete required trainings

 Prepare files for closure and storage

 Conduct Waiver Monitoring Processes

 Participate in Plans of Corrections

 Conduct the Individual Support Plan process

 Complete Service Preference process

 Process Requests for Services

 Complete Voter Registration form

 Conduct Pennhurst monitoring

 Complete Prioritization of Urgency of Needs Survey (PUNS) process

 Participate in the Supports Intensity Scale (SIS) Process

 Participate in the Independent Monitoring for Quality (IM4Q) Process

 Communicates current Office of Developmental Programs priorities to individuals and families

 Participates in the Positive Practices Resource Team (PPRT) process

 Newly hired Supports Coordinators will successfully complete ODP required SC Orientation Curriculum

 Complete a minimum of 24 hours of training per year

Minimum Requirements: (education, experience, etc.):

 A bachelors degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science; or

 Two years experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service; or

 Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.

 Computer skills to include word processing/working knowledge of Microsoft Office products

Minimum Employment Requirements:

 Automobile

 Valid Pennsylvania Drivers License and acceptable motor vehicle record check

 Have criminal history clearances per 35 P.S. §10225.101 et seq. and 6 Pa. Code Chapter 15

 Have child abuse clearance (when the participant is under age 18) per 23 Pa. C.S. Chapter 63

 Social Security Card

 Immigration and Naturalization (I-9) Clearance

 Drug screening clearance

 FBI Clearance

Working Conditions:

 In-office work performing concurrent varied tasks

 Daily travel throughout Lackawanna and surrounding counties in a variety of venues including but not limited to the community, individuals home, providers, hospitals and other human service agencies as well as occasional travel to statewide meetings.

Safety Hazards of the Job:

 Minimal hazards, general office working conditions. There may, however, be a rare occasion during which it is necessary to have physical interaction with a consumer exhibiting aggressive behavior.

 Some travel may be during adverse weather conditions.
