Our team works to help older and disabled adults feel independent at home, or wherever they reside. The care coordinator will be providing services to clients who reside in Luzerne, Lackawanna, Wayne, and Wyoming Counties.
The Care Coordinator functions as the liaison between Caregivers and Clients. A Care Coordinators primary responsibilities are to assign Caregivers to cases by making the best possible match for the Client and Caregiver and conducting home visits.
Makes contact with all active clients to check in on a monthly basis.
Conduct initial and follow up home visits. Work with client and clients family members during home visits to determine client needs and develop a care plan.
On call responsibilities on a rotating basis
Works the digital leads process for any digital lead, utilizing the 8-touch process.
Pulls and communicates payroll and billing data per the existing SOPs.
Understands Pennsylvania state regulations
Utilizes Outlook for email and calendar appointment/event scheduling.
Utilizes ClearCare and any other necessary systems for complete and accurate care coordination, Caregiver scheduling, data capture, tracking, notes and follow-up.
Must be able to independently drive long distances, sit/stand for extended periods of time and use standard business equipment including a laptop computer and a mobile phone as well as all the associated apps and software.

Necessary Attributes:
Strong verbal and written skills
Excellent computer skills, proficient with Microsoft office

Team player, and a positive attitude
Must handle on-call duties on a rotating basis and as needed by the office
Valid Driver's License, car insurance, and registration
Education: Preferred two years of experience preferably in a home care setting. We are a EOE.
