PRN Day/Evening/Nights Job Description: Under the general supervision of the Lead Central Alert Team Nurse or Director-Quality and Clinical Improvement, reviews, administers, coordinates and supports the System-wide 24/7 Central Alert system. Using advanced clinical assessment skills and knowledge with human response to life threatening problems, receives and screens care alerts by real time review of the patient electronic health record for early identification of protocol driven interventions. Utilizing nursing diligence, communicates for action as appropriate, related to alert and record identified order set and care protocol interventions. Works remotely and collaboratively by telemonitoring technology, acts as a real time clinical support for the HELP team, bedside nursing and/or physician/Advanced Practice Clinician on patient conditions that require immediate attention.Duties and Responsibilities Essential Accountabilities: Performs diligent review of electronic health record systems data and protocol driven alerts for screening of necessary and appropriate interventions, using clinical assessment and knowledge skill and pre-determined/defined order set(s).

Provides real time identification and communication of missed opportunity or care gaps related to alert.

Provides timely and appropriate communication of identified and related assessment to appropriate clinical staff and/or HELP team.

Prepares professional communication verbal/written of identified findings and interventions, including thorough and complete nursing documentation.

Maintains a thorough knowledge and understanding of evidence based/best practice for clinical care and related interventions.

Utilizes strong decision making, systems thinking and computer literacy skills in a remote practice environment, to assist in support of coordinated and timely patient care intervention.

Performs virtual “rounding” regarding patient assignment and alerts, reporting as directed.

Follows escalation process/procedure to address real-time care concerns.

Revises and monitors educational/training materials to ensure that they are coordinated, understandable and meet the program standards.

Protects patient confidentiality by promoting professional staff communications.

In downtime, follows a set procedure for review and decision making for several other job responsibilities including: incidental findings and the incidental findings registry as a care coordinator, and EWS alerts of various types.

NonEssential Accountabilities:

Responsible for the care and maintenance of department/entity equipment and supplies.

Attends meetings as required and participates on committees as requested.

Maintains ongoing education for licensure, certifications and as required within work specialization.

Performs other related duties as identified.

Qualifications

Other combinations of formal education, training and experience may be considered.

Minimum Experience:

2 years

Min Field of Expertise-Nursing experience in Emergency Department, Critical Care, Telemetry or sepsis specific setting

Minimum Education:

Diploma or Associate's Degree

Required Certification:

Certified Critical Care Registered Nurse (CCRN) or Certified Emergency Nurse (CEN) or other Professional certification

Physical Demands:

Mostly sedentary work

Occasional prolonged sitting/standing

Occasional reaching, stooping, bending, kneeling, crouching

Skills:
