The Registered Nurse (RN) is responsible for providing care and treatment for clients in their home environment or in a heathcare environment setting.
Provide 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence.
Directing and supervising the activities of the licensed practical/vocational nurses, CNAs, and caregivers as needed.
Monitoring clients vital statistics and reporting abnormal findings to the case manager and physician
Administration of medications, feedings, oxygen, ostomy care as needed per client's plan of care
Complete routines as established by other care professionals such as PT, OT, Speech Therapists
Ensure infection control policies are maintained at all times
Follow client's plan of care.
Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians and documentation of progress and outcomes for established goals
Informs physician, case manager, and families of changes in client's medical condition and needs
Ordering supplies, medications, and feedings as needed to insure adequate amounts are on hand at all times
Establishing and maintaining a professional relationship with families, clients, and other health providers
Report incidents or accidents to case manager
Ensuring all medical equipment is maintained and report faulty equipment to DME for repair
Performs other duties as assigned
Position Requirements
Registered Nursing Degree from an accredited college of nursing with current unrestricted registration and license in good standing from the state(s) of practice
Current CPR and First Aid professional certification certification
1+ years of nursing experience; supervisory experience, preferred
Home health care experience, preferred
Must be able to travel to, between, and from client's location, providing own transportation
Experience in administering medications preferred
Able bend stoop, push, pull, sit, and walk for periods of time and to lift and carry items weighing up to 20 pounds
