Griswold Home Care of the Lowcountry is in need of a Part-Time Care Coordinator. The Care Coordinator will work 9-5, 3days a week from the office and rotate on-call responsibilities. The position may be full-time for the right candidate. Care Coordinators are key personnel in the daily operations of a home care office. Care coordinators will interact with clients, caregivers and referral sources. Care Coordinator Duties:

Manage all client requests for initial, replacement and substitute caregivers

Conduct telephone intakes with new or potential clients

Facilitate productive dialogue between client, family, and caregivers

Refer and interact with appropriate caregivers

Manage and maintain client/caregiver scheduling software

Maintain files to meet all internal, state, and federal quality and compliance standards

Administrative work as needed

Evening and weekend 'on-call' responsibilities on a rotating basis

Office Schedule - 3Days a week - 9 to 5 - plus "on-call"

Essential Skills/Qualities:

Very strong communication skills

Nurturing ability (Caring, empathetic, compassionate)

Articulate, with strong verbal and written skills and pleasant phone manner

Understands teamwork

Welcomes accountability

Capable of prioritizing and multi-tasking

Organized and detail oriented; good documentation skills

Proficient with MS Outlook, MS Excel and smart phone technology

Capable of passing a background check

Prefer candidates in the Bluffton, Hilton Head area.
