Referral Coordinators with Griswold Home Care are key personnel in the daily operations of the home care office. From answering the phones, to being in the field interacting with clients and referral sources, care coordinators scope of responsibility is varied, fast paced, and rewarding. A mix of social services, operations, customer service, finance, sales, recruiting, and human resources are all parts of a Care Coordinator's exciting role. The mission of the company should coincide with his/her approach to care for the older and disabled population. Must be a compassionate, caring and competent individual with empathy and understanding.

Specific Responsibilities: Primary focus will be maintaining excellent Client relationships by assuring adequate scheduling, home visits, and case management.

• Support office administrator with scheduling, marketing, interviewing, and administrative duties.

• Possess the knowledge to conduct telephone intakes with new or potential clients.

• Perform initial home visit with new or potential clients to explain services. This in-home visit shall include review and completion of all required paperwork including but not limited to the Client Service Agreement, the Client Service Plan and other necessary documentation to ensure quality care.

• Conducts follow-up home visits every 45 days whereby satisfaction of services will be evaluated.

• Monthly phone contact with existing clients to ensure client satisfaction.

• Ensure the Client Service Plan is consistent with needs of the Client and making adjustments, as the client needs change.

• Report changes in the Client's condition to the family or responsible party, attending physician or other party according to the Policy on Maintenance and Security/HIPAA form signed by the Client regarding release of information.

• Resolve client, family, and caregiver differences.

• Maintain case notes and files to meet all internal, state, and Federal quality and compliance standards.

• Cooperate with other office personnel in the assignment of Caregivers to individual Clients. Select and interact with appropriate caregivers on a case-by-case basis.

• Coordinate all scheduling activities, including replacement and substitute caregivers.

• Assist with the orientation and training or supervising the orientation and training of caregivers assigned to provide personal assistance or companionship services as necessary.

• Assist in passing the annual Corporate Quality review and internal, periodic audits all the while, working towards achieving Platinum Heart status in the quality review.

• Conduct marketing and service calls with prospective clients

• Other tasks as deemed appropriate and necessary.
