The person in this position provides case management and training in skills that primarily focus on furthering an individual’s independent living, community skills and overall wellness and recovery. Services provided by the person in this position should promote community integration, increased stability within the community as well as an improved quality of life and implementation of self-determination for individuals served, as appropriate. Services provided by this position also include activities and skills-training designed to address barriers and strengths related to an individual’s recovery and wellness . This position is responsible for providing assessments and treatment plans for youth, completing documentation of services, assisting youth and their families with accessing other community resources as well as crisis intervention, as needed. This position is responsible for collaborating with community, families, doctors, and other agencies.
Salary: $20.04 - $21.79/hr
Location: Guadalupe County
Minimum Qualifications: Bachelor’s degree in human services field; ability to work as a member of a team within a fast-paced, treatment setting is essential to this position. Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment
Preferred Qualifications: Master’s degree and/or clinical license in psychology, education, social work or a related human service field; one year work or practicum/clinical experience in intensive case management and/or psychosocial rehabilitative services;
bilingual/Spanish-speaking; experience in providing concurrent documentation within an electronic database preferred.
