We have an opening for a PPS Coordinator.Choose to be an integral part of a team that helps patients achieve their greatest potential and work toward productive, independent lives. The PPS Coordinator is responsible for the review of medical records, diagnosis coding, data entry and submission of related information to the appropriate agencies, both federal and state. You'll work collaboratively with Nursing, Case Management and Physicians to ensure that accurate data is submitted.The PPS Coordinator exists to coordinate the data collection on the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI). Ensures the presence of supporting documentation and provides staff education to ensure compliance with CMS guidelines. Coordinates all efforts for immediate issue resolution concerning outcome scoring. Conducts ongoing review of IRF-PAI related data and completes or assists with the completion of the IRF-PAI.Employees in this position will:
Coordinates all areas of outcome score capture and supportive documentation for the completion of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI).
Provides initial and ongoing outcome scoring and PPS education including new-employee orientation and focused education as needed.
Tracks outcome training completion and functional outcome competency of clinicians.
Reviews medical records to assure that clinical documentation supports outcome scores. Confers with the clinical team members regarding the scoring of section GG items to assist in resolution of any inconsistencies.
Reviews medical records and confers with medical staff members to assure that co-morbidities are properly identified and documented in the medical record.
Confers with the patient and clinical team members regarding the scoring of section GG items, follows up on any inconsistencies, and, when necessary, assesses the patient to confirm the correct score.
Reviews all Rehabilitation Impairment Categories and admission ICD-9 codes to assure congruence and proper Coding for the IRF-PAI. Makes any necessary changes in collaboration with the clinical team and physician.
Works directly with any clinical department to focus on documentation trends and corrective action to assure that the Medical Record documentation supports the IRF-PAI documentation.
Continuously assesses FIM scoring and FIM documentation competency and provides focused FIM training as necessary.
Collaborates with the PPS Manager in conducting audits and reviews and prepares reports as assigned.
Assists the PPS Manager with PPS functions including but not limited to: IRF-PAI data collection, medical coding, IRF-PAI transmittals, and IRF-PAI correction.
Based on medical record review, assists in compiling data, tracking trends, and provides feedback to the clinical team regarding documentation, including providing input as to staff training needs.
Participates in performance improvement initiatives and staff education to ensure that staff is provided the tools and support they need to accurately document outcome scores and co-morbidities according to CMS standards and guidelines.
Provides coverage for PPS in the PPS Manager’s absence.
Participates in PPS and outcome related committees and meetings as assigned.
According to need, provides coverage at other sites for PPS coordinators.
Minimum Skills, Knowledge & Abilities (including licensure, certification and other job-related credentials)
Current valid state licensure in Nursing or allied health profession required.
Minimum of two (2) years of previous experience in an inpatient rehabilitation or closely related setting required.
Experience with FIM’s and IRF-PPS preferred.
