The Program Coordinator will coordinate the delivery of a tobacco cessation intervention through phone-based, manualized counseling and coordinate the logistics & reporting of patient/provider interactions associated with the CPRIT funded Tobacco Prevention and Cessation Program (TNET) in Texas, which may be during regular, evening and weekend hours. This position may require travel. This position requires hybrid onsite and remote work. Details can be found in the competencies section and the working conditions section of this Position Description.The ideal candidate will have experience in community project coordination and bilingual in English/SpanishJOB SPECIFIC COMPETENCIES:Counseling and Enrollment Coordination Coordinates patient enrollment and cessation counseling activities among project team members in Houston and Tyler regions. Coordinates the logistics for patient interactions, including but not limited to, establishing quit dates; organizing, scheduling and documenting 11 counseling sessions per patient; assessing tobacco use history; educating patients on Nicotine Replacement therapy; providing brief quit advice; and coordinating the preferred cessation intervention. Coordinates and conducts counseling sessions serving Northeast Texas patients. Serves as a liaison on behalf of the program with collaborating clinics, staff and legal services as directed by supervisor. Trains staff to help perform duties appropriately based on Northeast Texas culture, tobacco tendencies, interventions preferred, and experiences with cessation counseling sessions. Advises staff on program process changes for the aforementioned areas. Provides feedback to staff and leaders for performance adjustments and evaluations.Management of Records, Documents and Data Coordinates data gathering for specific documentation, record keeping, data entry and reporting, regularly or on demand from team leadership. Collaborates with manager on preparation of quarterly funding reports and presentations. Maintains database for the patient counseling services. Coordinates development and implementation of various databases for program services (CME, smartphone app and cessation). Shares information with leadership to review staff and operational readiness. Attends, participates in and reports at meeting to improve program services, troubleshoot database problems and text intervention challenges. Prepares counseling reports on data collection representative of program's progress and accomplishments for presentations within the institution and the Tyler region's collaborators.Administrative Coordination - Program Logistics Coordinates administrative tasks such as processing of institutional documents (requisitions and forms); acquisition of resources, services and tangible assets; scheduling meetings; and maintenance of all program logs related to counseling activities and patient contact. Develops and drafts written standardized procedures for cessation counseling services. Evaluates daily operational tasks for process improvements enhance patient services. Recommends operational revisions and coordinates implementation of revisions onsite and remotely. May substitute required education degree with additional years of equivalent experience on a one to one basis. With preferred degree, one year of required experience.Preferred: Experience in community project coordination. Knowledge of behavioral and psychosocial determinants of health-related behaviors. Excellent interpersonal and communication skills.
