Job Summary
The Registered Nurse II-Home Health/Hospice with the collaboration of physician and healthcare team is responsible for planning, organizing and directing a care plan for home health or hospice patient care.

Job Description
Full time, Mon-Fri & rotating weekends as needed
Southeast Region covers: Downtown Houston, League City, Pearland, Lake Jackson, Galveston, Channelview, Manvel, Rosharon and Baytown (area is divided into teams by zip codes)
Southwest Region covers: 50 mile radius from Stafford, TX which includes Missouri City, Sugar Land, Rosenberg and Needville (area is divided into teams by zip codes)
Texas RN license and BLS required
1+ years RN experience required
Home Health experience preferred
Must have reliable car and car insurance
Minimum Qualifications
Education: Graduate of an accredited School of Professional Nursing
Licenses/Certifications:
