Job Description

New Heights is seeking a Care Coordinator - Marketing. Under the supervision of the Care Coordinator Lead, the Care Coordinator will: Build & maintain healthy relationships with referral sources to obtain new clients. He/she will provide assistance to new potential clients seeking care, including registering the client, explaining services and benefits of said services to clients and other referral sources. He/she will perform other functions of the office, including scheduling, responding to telephone inquiries, verifying active insurance coverage and etc.

Key Responsibilities & Duties:

Marketing & Outreach

Participate in community events and meetings to recruit clients, identify resources, and build and maintain strong partnerships with community groups, clinics, collaborative groups and other entities serving all ages individuals to enhance mental health issue awareness and knowledge.

Distribute mental health/mental illness flyers, brochures, community resource information, and other materials to community members, organizations serving families, seniors, or community individuals and groups that can spread the word to potential participants in the city(ies) assigned, to obtain enrollments into the program.

Provide Community presentations discussing crisis intervention, community resource referrals, and education on services offered at New Heights Community Support.

Capture, document and track all client recruitment activities in the program/department database.

Comply with outreach goals set by the supervisor.

Maintain client volume, by keeping current with changing trends, economic indicators, and competitors.

Maintain/Learn professional and technical knowledge by attending educational workshops; reviewing/read professional publications; establish personal networks; participating in professional societies.

Contribute to team effort by accomplishing related results as needed.

Care Coordination

Responsible for new intakes and performing intakes of new clients seeking care.

Assist clients with the client application process, registration, and appointment scheduling.

Assures accurate and timely collection and data entry of all required client information, insurance and other information.

Provides assistance with other functions of the business office, including: client check- in’s, responding to telephone inquiries, verifying active insurance coverage, and other related office duties.

Maintains patient confidentiality and adherence to HIPAA regulations.

Qualifications

High School Diploma

1-2 years of experience in a community mental health setting.

1-2 years of Marketing experience.

Valid Driver’s License.

Knowledge/Skills:

Ability to coach and manager processes.

Ability to develop and adhere to budgets.

Possesses good written and oral communication skills.

Ability to work independently, plan and organize daily work.
