Senior Healthcare is seeking LPNs with Home Healthcare experience to case manage patients in

their homes in the Lynchburg and surrounding areas.

We are seeking LPNs with the following attributes: Professional in conduct and appearance, self-starter,

Problem solver, organized, compassionate, strong communicator, understands family dynamics.

Responsibilities:

Provide best in class complex care management to the most vulnerable patients

frail elderly, the chronically and functionally challenged.

Identify risks, gaps in care and prevent unnecessary hospitalizations and emergency room

visits by developing an individualized interdisciplinary plan of care for members and families

to follow in order to achieve lifelong well being in the home.

Educate the patient on preventive healthcare

Maintain HIPAA compliance

Educate on resources to assist with medication reconciliation and understanding of medical conditions

Comply with all on boarding, annual and other mandatory training's as assigned.

Job Duties

In home visits with patients

Reconciling prescribed medication

Assessing patients environmental, functional, psycho-social status and financial well being

Identifying and developing action plans to empower members to remain and thrive at home

Connecting patients and their families with approved resources, services and professional intervention who

can help them address medical, legal, housing, insurance and financial issues.

Completing required paperwork and documentation within the time frame required

Collaboration with other disciplines and participation in interdisciplinary team including physicians

Requirements:

Licensed Practical Nurse with Valid Virginia or Compact License

Minimum of 3 years of relevant experience with at least one year of Home Health experience or Community

based or In-Home Visit experience.

Experience working with either Geriatric, chronically ill or functionally challenged populations

Excellent Computer skills to include: Ability to use electronic information application, recent EMR

experience

Valid Drivers License with clean record, car insurance and access to an automobile for home visits is

required.

Ability to pass Criminal Background and Drug Screenings

Valid PPD
