Looking for psychiatrist to talk to and fill out DMV form that says I am okay

Looking for for a psychiatrist to talk to me on the phone or video chat and verify I am okay, then, I can email you a DMV form that they want 2 simple pages out so a psychiatrist says I am mentally okay to drive a car. I have never had my mental health impair my driving. But the DMV wants a psychiatrist to say it on one of their forms so they can unsuspend my license. Thank you.
