About the Open Positions at Augusta Health
We have immediate openings for: RN Care Manager–Home Health, RN–Home Health and LPN–Home Health
Home Health Caseload: Approximately 40 patients; 5 to 6 calls per day
RN Care Manager-Home Health
A registered professional nurse responsible for managing and coordinating the care of a group
of patients for their entire home care stay, utilizing all aspects of the medical plan and
nursing standards of care. Directs and guides patient/family education and the activities of
other personnel while maintaining standards of professional nursing. Must possess the ability
to assess data reflective of the patient’s status and interpret the information to formulate a
plan of care based on the patient’s age-specific needs. Must be able to provide the care
needed as described in the department’s policies and procedures.
Requirements
Virginia State RN License and Virginia State Driver’s License
1 year of recent Medical/Surgical experience; 1-year Home health or hospice experience
IV therapy, wound care, general medical/surgical assessment skills
RN-Home Health
The Registered Nurse provides skilled nursing care for home health following physicians’ orders and the established Plan of Care. This position participates in the supervision of Home Health Aides and LPN’s. Coordinates and collaborates with the interdisciplinary team ensuring the enactment of the Plan of Treatment. Reports to the Clinical Coordinator.
Requirements
Virginia State RN License and Virginia State Driver’s License
1 year of recent Medical/Surgical experience; Home health or hospice experience preferred but not required
Physical assessments, IV therapy, wound care, communication, organization and documentation
LPN-Home Health
Provide direct patient care in home care setting. Work under the direction of a registered nurse. Collect data, administer medications, document and perform nursing procedures competently as ordered.
Requirements
Licensure as an LPN by the State of Virginia; CPR certified
Medical/Surgical experience in hospital or LTC setting within the last year; Elderly care and care of the terminally ill a plus
