Location: Bellingham/Whatcom County
Caseload: 10 people in a short term transitional program
Program: Geriatrics Transition Program (GTP)
Duties: work with older adults with mental health problems, physical health problems and challenging behaviors. Provide in-depth assessments of clients, their behaviors and work with them and their caregivers to resolve conflicts and stabilize their living situations.
Requirements: Master's degree in Social Services field, personal car, liability insurance and a valid WA driver's license.
