SE WA AGING & LONG TERM CARE COUNCIL OF GOVERNMENTS has a fulltime 40-hr-per-week Mon-Fri 8-5 Case Manager opening in Kennewick who will assist adults with disabilities, older persons and family caregivers to assess needs, screen for and assist persons to arrange needed in-home & community services or authorize and maintain in-home & community-based services, including services to improve health of high-needs high-risk clients.
Experience & Training: Bachelor's in Social Work, Social Science or related field AND 2 years direct service case management or social service program experience required (Paid or volunteer in social service agency). Experience meeting and working with people in homes and other medical or community settings. Bilingual Spanish preferred. Applications accepted until Noon August 7, 2019.
