JOB FUNCTIONS: Community Choice Guide (CCG) / Client Trainer (CT)

JOB PAY: $18-22+ Per Hour to Start Depending on Experience

JOB SUMMARY:

The Independent Living Specialist will serve as a member of the Validus Consulting and Services team to facilitate individuals with disabilities and age-related impairments in maintaining, advancing, and maximizing their independence. This work will be accomplished through multiple avenues, including but not limited to the following:

I. COMMUNITY CHOICE GUIDE

A. Transition Services:

The Independent Living Specialist (ILS) performs transition services. In so doing, the ILS:

1. Works with case managers and staff of Home & Community Service (HCS) and Area Agency on Aging (AAA) to evaluate and implement the relocation of individuals living in institutional settings.

2. The ILS provides intensive one on one relocation and transition-related support for individuals moving from qualified institutional settings to qualified community settings as authorized by case management staff in the participant's Service Plan.

B. Transition Services Job Description:

1. Conduct Pre-Transition Consultations with clients to determine support and/or services needed for successful relocation and transition: Identifying specific needs along with available resources, noting any potential or perceived barriers while determining a course of action for coordination of services and implementation of strategies to eliminate or overcome obstacles

2. Act as a liaison on behalf of HCS and AAA staff between nursing facility and in-home case management, staff at nursing facility and institutional settings, housing, medical personnel, legal representatives, formal caregivers, family members, informal supports and any other involved party

3. Provide assistance and support to ensure the participant's successful transition from the institutional setting as authorized by HCS and/or AAA staff. This could include transfer of medical prescriptions, processing change of address and facilitating the acquisition of furnishing and other essential household items for example.

4. Execute stated duties and others that are unstated but may be assigned in accordance with the mission, values, and established Policies and Procedures of Validus Consulting and Services in accordance with while demonstrating a high-level of client-centered service.

II. CLIENT TRAINER

A. Client Training Services: The Independent Living Specialist (ILS) performs client training. In so doing, the ILS:

1. Works with case managers and staff of Home & Community Service (HCS) and Area Agency on Aging (AAA) to evaluate clients' needs and implement training services related to and required for clients to maintain and maximize their ability to live independently

2. Assists and facilitates clients in structuring their living circumstances and developing their skills necessary to carry out the essential daily tasks of self-care/self-direction and required to 1) remain living at home and to do so as independently as possible and 2) engage in community life as fully as possible.

B. Client Training Job Description:

1. Act as a liaison on behalf of HCS and AAA staff between nursing facility and in-home case management, staff at nursing facility and institutional settings, housing, medical personnel, legal representatives, formal caregivers, family members

2. Performs independent living assessments which involves facilitating clients in identifying their personal/vocational goals within the larger context of their related circumstances.

3. Develops independent living plans which involves facilitating clients in documenting their IL related goals, identifying the major barriers preventing advancement toward those goals, and developing strategies and steps with clients to eliminate those barriers.

4. Facilitates clients in implementing their IL Plan, as well as identifying, contacting, and engaging existing community resources necessary to eliminate barriers and advance toward greater independence.

5. Execute stated duties and others that are unstated but may be assigned in accordance with the mission, values, and established Policies and Procedures of Validus Consulting and Services in accordance with while demonstrating the highest level of quality client-centered service.

III.QUALIFICATIONS / REQUIREMENTS (Non-negotiable)

A. Education and/or Experience

1. Bachelor' degree in social work or psychology with two years of experience in the coordination or provision of independent living services (ILS)

OR

2. Two years of experience in the coordination of ILS in a social service setting under qualified supervision;

OR

3. Four years personal experience with a disability.

B. Reliable Transportation / Current WA State Driver's License & Auto Insurance

C. Computer access and proficiency: Ability to access and exchange electronic transmissions on a daily (timely) basis. Desire experience/proficiency with Microsoft Office and Outlook.

D. Excellent verbal and written communication skills

E. Strong organizational skills, ability to coordinate and multitask

F. Self-starter: able to prioritize and execute duties with minimal oversight

G. Otherwise conduct yourself in accordance with the mission/values of and in compliance with all established Policies and Procedures of Validus Consulting and Services while demonstrating the highest level of quality client-centered serviceĀ¦

1. Within VCS among VCS staff,

2. Between VCS and all other private and community organizations, program/service providers, vendors, etc.,

3. Between and within VCS and our contractual relationship with the holders of our contracts, e.g., DSHS-AAA and all their representatives, representatives and staff including case managers, supervisors, etc.

4. Within the general public, i.e., During working hours AND any other time you are representing or are otherwise conscious of being viewed as a representative of VCS.

WORK AREAS: Staff is currently being hired to serve each of the following areas: -Whatcom, -Skagit, -Island, -Snohomish, -Pierce, -King, -Kitsap, -Thurston, -Mason, -Lewis, -Clark Counties
