The Case Manager is responsible for the intake, evaluation, coordination and provision of services to individuals seeking services. This includes facilitating 1:1 interviews and case management sessions to determine client needs, identify barriers, and formulating a plan to address needs and provide assistance with barrier removal solutions.
EXAMPLE OF DUTIES:
Performs in-depth intake and evaluation with participants utilizing applicable tools to determine eligibility, including the gathering of clinical and demographic information.
Performs in-depth initial vocational assessment and interviews to determine existing strengths, abilities, and interests of clients.
Initiates individual case management plans with each participant outlining participants short and long-term goals based on their perception of needs and provides guided assistance in accessing applicable community resources.
Acts as a liaison between clinical staff, community partners and other community providers.

Monitors and modifies the meeting of participant goals, performs and documents intensive 1:1 case management sessions, reports concerns and findings
Generates monthly reports in relation to the meeting of project goals/performance standards
and consults with mental health support team regarding the outcome of reports and the facilitation of any necessary performance modification.
Models respect and compassion to all participants and utilizes motivational interviewing and harm reduction principles and techniques to minimize resistance and encourage participant change.
QUALIFICATIONS:
Candidate must have a bachelor's degree, preferably in a human service field and current Agency Affiliated Counselors license or the ability to obtain one and supervised practical experience in a clinical or rehabilitation (recovery) setting with persons with severe and persistent mental illnesses.
