The Community Resource Specialist functions as an important link between the patient's primary care team and their surroundingcommunity and empowers patients to get active in their own care. This role develops relationships with community organizationsand connects patients with community resources. The Community Resource Specialist conducts interviews to help identify thepatient's health goals, their desire and readiness to change behavior and helps the patient create a plan to take action towardmeeting their health goals. The Community Resource Specialist also helps patients navigate through the Kaiser Permanentesystem.Essential Responsibilities:Coaching patients with low to medium care needs to identify health goals and remove barriers to meeting goals.Work with patients around goal setting, action planning, problem solving, and follow up. Coaching may be telephonic,face to face, and/or via virtual consultations. Document services provided in the electronic medical record.
Actively linking patients to community resources to better meet the patient's specific needs.
Developing contacts in the local community and maintaining ongoing relationships within the community. Includesattending select community meetings, coalitions, and events, which may occur evenings or weekends.
Act as an active member of the primary care team, including educating and informing staff of local resources.Interact with other Kaiser Permanente departments to ensure coordination. Work with volunteers to support patient andcommunity needs.
Assisting with internal patient navigation needs.
Basic Qualifications:ExperienceMinimum one (1) year of experience providing direct services to clients.
EducationHigh School Diploma or General Education Development (GED) required.
License, Certification, RegistrationWA Drivers License by date of hire/transfer
Additional Requirements:Experience interacting in a diverse, multicultural and/or multilingual environment.
Computer literacy including Word, Windows, Excel, and Social Media.
Understanding and sensitivity to how socioeconomic, environmental, cultural, and other factors influence health.
Ability to participate and collaborate in a team setting.
Good organizational skills.
Strong communication skills.
Ability to multi-task.
Self-motivated.
Preferred Qualifications:Two (2) years of working in a community service organization
Familiar with and active in local community.
Bi- or multi-lingual.
Some college preferred.
