The Chippewa County Department of Human Services presently has an opening for a part-time position within the Aging and Disability Resource Center. This position will be primarily responsible for administering the Adult Long-Term Care Functional Screen for enrollment into the FamilyCare and/or IRIS programs. The position averages 19 hours per week and requires home visits, extensive documentation and reviewing medical records.
Minimum Qualifications: Bachelorâs Degree in a Health or Human Services related field required. Adult long-term care functional screening experience preferred. Wage: $23.61 - $29.74 per hour depending on qualifications. This is a non-benefited position. Position may be eligible for an annual pay increase based on performance.
