Capitol Lakes, a vibrant and beautiful senior living community in the heart of downtown Madison and a part of Pacific Retirement Services, has an immediate opening for a full-time nurse to serve as a Resident Care Manager (RCM) who works to ensure the delivery of person-centered, individualized and clinically competent care. The RCM coordinates and manages the overall care delivered to residents, temporary and permanent, in the Health Center, assuring that services are appropriate to meet resident?s needs, in accordance with Federal, State and local regulations from admission to discharge. A minimum of an associate's degree is required; BSN is strongly preferred. Some experience in a long term care (LTC) or Skilled Nursing Facility (SNF) is desirable. Charge Nurse/Supervisor experience preferred. Discharge planning and/or Case Management experience a plus.
