JOB REQUIREMENTS: Person-Centered Planning Ensure that each TEAM meeting is participant driven and that meeting outcomes reflect a cooperative process with participant directed goals and meaningful outcomes. Case managers are also responsible for making sure that at the TEAM meeting the focus person feels comfortable, listened to, understood, respected, and that their dignity is maintained. Coordinate and facilitate (when requested by the focus person) TEAM meetings including scheduling, sending invitations, making physical arrangements, refreshments, and follow-up correspondence including documentation of minutes from the meeting and the writing of new program goal(s) and documentation forms. Prior to each TEAM meeting, conduct personal interviews/surveys with each person to gather information, review progress and to determine what they view as their successes, obstacles and future dreams. Also, modify (when necessary) inappropriate/ineffective program plans and/or goals. Invite/encourage participant and stakeholder input regarding satisfaction with agency services via distributing satisfaction surveys to all TEAM meeting attendees. Objectively advocate for the person's rights to fully participate (to the best of their ability) in the TEAM process. Work cooperatively with all parties associated with assigned participants to creatively tie vocational, prevocational, and/or personal enrichment goals into what the person has identified as their preferred action plan and help them understand that there may be limits (financial, safety, program related etc.) on actualizing their dream(s). Case Management (Social Work) When requested by the department manager, counsel and/or support assigned participants regarding human development issues, behavioral challenges, participant rights or advocacy issues and/or during difficult times or medical emergencies. Write proactive and respectful support plans for assigned participants that are designed to proactively modify challenging behavior(s). When requested by outside parties, analyze and report on assigned participant data. Facilitate and complete the intake, exit or transfer process for assigned participants. Including conducting main file set up, and long-term maintenance of, and procurement of necessary family care or county and/or guardian authorization, releases and permission statements. Case managers are also responsible for facilitating a meeting with program personnel to familiarize them with the incoming person while stressing confidentiality of information. The position is also responsible for coordinating participant exit meetings, completing and disseminating an exit report, and for closeout of the participant's main files upon the person leaving the agency. Serve as key communication point for assigned participants' families, family care/Community care case manager's and/or caregiver(s). Professional Submission of quarterly reports to the Rehabilitation Director. Consistently represent the agency in a professional fashion. Market the organization in a manner that increases agency-wide participant referrals. Completion of all written correspondence/reports in a proficient manner. OTHER EXPERIENCE AND QUALIFICATIONS: Professional Skills and Abilities: Person-Centered Planning: Knowledge of and respect for the person centered planning process including an understanding of how to educate and empower people regarding their right to exercise self-determination and/or self-advocacy. Ability and willingness to recognize and celebrate a person's successes and unique abilities while defining limitations in a sensitive and respectful manner. Ability to exhibit and maintain a flexible, receptive and non-judgmental demeanor that encourages creative thinking and collaboration. Ability to objectively and creatively present and offer choices to participants. Case Management Related: Ability to write measurable, clear, reasonable, and concise participan t focused goals, outcomes and comprehensible methodologies. Demonstrate creative problem solving and networking skills. Ability to develop, write and/or implement meaningful, proactive and positive strategies for supporting participants with challenging behaviors and/or human development issues. Professional Skills: Computer skills. Ability to plan, organize, prioritize and efficiently complete multiple responsibilities within established time frame. Good communication and interpersonal skills. Ability to consistently and diplomatically work on a professional level with key stakeholders and people from different departments within a complex organization.
