Job Description: Outreach Worker

Full Time Hourly

Hourly Rate: $15.00-18.00 per hour (Depends on Experience)

BILINGUAL IN ENGLISH/SPANISH REQUIRED

SUMMARY: Performs outreach/intake (registration), education and enrollment duties necessary to serve the Agency’s clients properly.

GENERAL MISSION STATEMENT: To improve the health status of migrant and seasonal agricultural workers and their families.

ESSENTIAL KNOWLEDGE, SKILLS AND RESPONSIBILITIES: The following are intended to illustrate typical duties; they are not meant to be all inclusive or restrictive. Other duties may be assigned.

  • Completes a registration packet on person to determine eligibility for programs available through agency or by referral.
  • Assess medical, dental, behavioral and social service referrals.
  • Will become familiar with the Affordable Care Act and will successfully complete all required components of Outreach and Enrollment activities and training applicable to become a Certified Application Counselor.
  • Coordinates effectively with clinicians to follow-up with patients regarding clinic referral visits and patient compliance.
  • Must be able to convey basic health information provided by the clinician to the patient and to pass on to the clinician the patient’ understanding and questions.
  • Must be able to translate (if bilingual) and transport client to any referrals made by the clinicians as needed.
  • Must submit a work plan weekly identifying tasks to accomplish and will submit a document the following week documenting results.
  • Must participate at staff meetings, outreach meetings and O & E meetings, webinars and other meetings as deemed necessary for position.
  • Must be able to perform general office duties such as typing, filing, receptionist duties and light housekeeping.
  • Must be able to effectively use technology and computer software, i.e., internet, email, MS Word and Excel at moderate or high level.
  • Must know how to use office equipment, such as copy machine, fax machine, printers, etc.
  • Uses time wisely and work well independently as well as working as a team member.
  • Will be expected to do some lifting of supplies and equipment.
  • Must be flexible to do a variety of jobs.
  • Maintains confidentiality of clients and employee related internal matters. (HIPAA)
  • Must have excellent reading comprehension, written and oral communication skills.
  • Follows instruction and document work.
  • Must do paperwork completely, accurately and on time.
  • Must have a car, valid driver’s license and liability insurance and able to travel overnight.
  • Must be able to establish a working relationship with patients, fellow employees, contracting providers, other agencies, organizations, organizations and the public.
  • Must be able to work, with limited supervision.
  • Must be conscientious of the quality of his/her work.

Qualifications to perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

EDUCATION AND EXPERIENCE:

  • High school diploma or GED and one year of office or public contact work and
  • Any combination of experience and training that provides the equivalent scope of knowledge, skills and abilities necessary to perform the work.
  • Bilingual in English-Spanish required

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Certified Application Counselor as applicable.

SUPERVISORY RESPONSIBILITIES: None

SUPERVISION RECEIVED: Immediate supervision from Area Service Coordinator.

SPECIAL NOTE: Our clinical staff trains the Outreach Worker staff on an on-going basis, on basic preventative health educations, so the Outreach Worker staff can reinforce to our patients, positive life changing practices to improve health outcomes.

COVID-19 considerations:
Employees are screened daily when reporting to work and required to monitor and report health status. Employees are required to wear face mask and other PPE as may be required in direct client/patient contact.
