The purpose of this scope of work is to obtain the professional services required by state law to
provide Emergency Medical Services (EMS) medical direction for the BLM-WY EMS Program.
The BLM-WY EMS Program will provide emergency medical care for wildland fire operations,
law enforcement activities, and employees involved in remote work projects.
The Medical Director must be a Wyoming licensed physician and will have overall
responsibility for the medical aspects of care provided by BLM medical providers in the state.
Failure to produce a current and valid Wyoming Medical License will disqualify candidates from
considerations.
This includes but is not limited to assistance and expertise with the development of medical
supervision plan, provider credentialing, assistance with protocol development and maintenance,
continuing educational activities, patient care review, and contemporaneous on-line medical
control.
This contract is for one base year and four option years that may be exercised based on available
funding and need. Exercising of option years is solely at the discretion of the government. It is
estimated at approximately 80 hours average annually depending on fire activity to fulfill the
needs of this requirement.
Medical Director Services shall include:
1. Training: The contractor will be required to provide and attend annual EMS Program
affiliation training in conjunction with BLM-WY. BLM-WY will coordinate a single
location and time for this training to occur. The contractor will provide input for
training which will cover the dynamics of emergency medical care for wildland fire
operations, law enforcement activities, and for employees involved in remote work
projects. During this training the contractor will cover the medical supervision plan and
medical protocols for the BLM-WY EMS Program. Travel and per diem expenses will
be inclusive of the contract payment, and therefore will be covered by the Medical
Director.
2. Program Development and Maintenance: The contractor will provide consultation,
direction, and expertise to the Contract Officer Representative (COR) on the
development and maintenance of the EMS Program for BLM-WY. This will include
maintaining program status as a Wyoming Fire Protective Service EMT licensure,
continued development of personnel licensure and certification, credentialing and
deployment of medical resources on initial attack fire responses, law enforcement
operations, and work projects in remote areas. This plan will also address: emergency
medical evacuations and destination criteria, tactical medical training, and AED
guidelines for remote worksites.
3. Protocol Review and Approval: The contractor will assist BLM-WY in providing
oversight for the established program protocols and will function as the Medical
Director who will review and approve the protocols every year or as necessary. The
contractor will also provide Extended Scope of Practice documentation for medical
protocols on an as needed basis.
4. Subject Matter Expert (SME): Provide guidance on up-to-date medical issues attained
through participation in local, state, national meetings, workshops, and continuing
education that impact the quality of care provided in wildland fire incident or remote
wilderness location settings.
5. Education: Provide oversight, develop content, approve practical exercises and certify
the 72 hour minimum state continuing education bi-annual EMT educational meeting/
training as requested.
6. Patient Care Report (PCR) Review: Review an adequate sample of PCRs for each
incident to determine quality of documentation for problem assessment/reassessment,
treatment, and disposition. Based on this review, provide a written response with
recommendations such as a change in documentation or treatment protocol and topics
for continuing education.
7. Online Medical Direction: The Medical Director will be available to answer questions
and provide direction to the EMS providers in the field. The Medical Director, or
approved designee, will provide a response within 10 min from initial contact from the
months of April through October.
8. Liability Insurance: The Medical Director will maintain and provide proof of liability
insurance on an annual basis to the Contracting Officer (CO).
9. Quality Assurances Performance Plan (QAPP): The Medical Director will submit
monthly written reports to the COR during the identified fire season and quartile reports
outside of fire season including;
a. Summary of all submitted wildland fire Patient Care Reports (PCR)
b. Review on 10% of: submitted PCR, incident accident sites, summary of SME
activities, and online medical direction requests.
