SUMMARY OF POSITION: The Patient Navigator (Case Manager) provides field-based case management services to patients enrolled in the Ryan White program. This person is the main point of contact for patients. The Patient Navigator builds strong relationships with HIV-positive patients in order to help patients stay engaged in medical care and adhere to their medications. Patient Navigators are committed to removing the patients barriers to care by identifying critical resources for patients, helping them navigate through health care services and systems, and promoting patient health. They work closely with the Care Team, which may include doctors, nurses, nurse case managers, and other clinical staff to support positive patient health outcomes.
QUALIFICATIONS: Minimum of Bachelors degree in Social Work, Human Services, Psychology, Public Health, or a related field. Minimum of two (2) years case management experience required. Experience in primary care or public health highly desirable. Knowledge of specific disease and lifestyle related topics such as HIV/AIDS and Hep C preferred.
Must be able to drive personal car for work-related activities, have current driver's license, proof of automobile insurance and clean driving record. Must provide current motor vehicle record. Mileage reimbursed at current IRS business mileage rate.
