Prepare, arrange, monitor, and record information for authorized patient/escort travel. Essential Patient Referral/Travel Coordinator Job Functions COMPLETES REFERRAL PROCESS BY Ø R eviewing Referral form to determine patient care service required; Ø Confirming patient is eligible for contract health services; Ø Confiming alternative resource procedure has been completed; Ø Scheduling appointment(s) with identified provider for patient; Ø Preparing necessary medical records as indicated; Ø Ensuring that medical records have been transmitted/transported to approirate receiving agency prior to scheduled appointment; Ø Coordinating patient travel requirements; Ø Signing referral form indicating external patient care service(s)/travel authorized. GUIDES PATIENT TRAVEL BY Ø Clarifying travel preferences, answering inquiries, and offering suggestions; Ø Training other staff positions to identify and book the most cost-effective travel options including but not limited to airline, transportation, and lodging reservations; SCHEDULES ITNERARY BY Ø Determining travel dates and destination, mode of transportation, finances, and emergency contact information; Ø Identifying, booking, and confirming route, carriers, and lodging; Ø Verifying space and rates. PREPARES TRAVEL PACKET BY Ø Assembling itinerary, tickets, confirmations, and related documents; Ø Explaining additional travel costs not provided such as cab fare, meals and incidental expenses; Ø Distributing to patient or designee. MONITORS PATIENT SERVICES/TRAVEL BY Ø Informing patient and provider contact information for referral assistance as needed; Ø Providing appropriate provider orders; Ø Coordinating follow-up appointments; Ø Documenting all patient services scheduled on PCC form; Ø Notifying appropriate staff of referral appointment changes; Ø Completing and filing referral form after services complete. Essential Administrative Duties MAINTAINS PATIENT TRAVEL RECORD BY Ø Collecting travel authorizations with travel dates, authorization numbers, patient/escort names, diagnosis and cost information; Ø Entering pertinent information in the RCIS software program. COLLECTS TRAVEL PAYMENT BY Ø Entering and verifying scheduled services and charges; Ø Computing total; Ø Obtaining credit card charge; Ø Obtaining refunds and adjustments as necessary. ASSISTS SUPERVISOR BY Ø Providing assistance to coworkers as necessary; Ø Assisting in training of new employees; Ø Completing staff payroll timekeeping duties; Ø Assuming supervisory duties during his/her absence. MAINTAINS OFFICE SUPPLIES INVENTORY BY Ø Checking stock to determine inventory level; Ø Anticipating needed supplies and associates costs; Ø Placing and expediting orders for supplies; Ø Verifying receipt of supplies. MAINTAINS SAFE WORKING ENVIRONMENT BY: Ø Reporting any unsafe conditions to appropriate supervisor upon discovery; Ø Adhering to all local, state, and federal health care compliance regulations; Ø Complying with federal and state laws and regulations regarding the protection of the health of employees and provide for appropriate occupational health services for those employees. MAINTAINS QUALITY IMPROVEMENT PROCESS BY Ø Understands the Tribe’s and employees needs by demonstrating courtesy, diplomacy and tact during interactions with internal and external parties; Ø Prepares and monitors indicators for quality and continuous improvement; Ø Demonstrates continuous initiatives for improvement of contract health services; Ø Takes a systems approach to problem solving and process improvement, which involves staff. CONTRIBUTES TO TEAM EFFORT BY: Ø Participating in the appropriate patient care services committees and other workgroups; Ø Initiating and directing training programs and in-services for new employees; Ø Performing other related duties as directed by supervisor. MAINTAINS PROFESSIONAL AND TECHNICAL KNOWLEDGE BY Ø Attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices. Additional Job Duties : Ø Works with providers to improve their knowledge about referral services appropriately; Ø Develop and maintain cooperative relationships between health care facilities and other agencies to assist in exchange of patient care service information. Ø Recognize, identify, and resolve problems and issues without direct supervision; KNOWLEDGE, SKILLS, AND ABILITIES Ø Knowledge of office equipment and limited maintenance; Ø Knowledge and ability to carry out rules of Health Insurance Portability and Accountability Act (HIPAA), which protects patient rights; Ø Skill of being able to utilize a maximum degree of professionalism and diplomacy with those who may be experiencing a high degree of stress mentally and/or physically (for both patients and co-workers); Ø Skill in operating a personal computer utilizing a variety of software applications including but not limited to RPMS and RCIS programs; Ø Ability to collect data, keep accurate and organized records, and ensure timely preparation of reports; Ø Ability to communicate professionally over the telephone and in person in a positive and clear manner; Ø Ability to manage simultaneously multiple tasks, including telephone, computer operations and in-person visits; Ø Ability to communicate professionally over the phone and in person in a positive and clear manner; Ø Ability to work independently with minimal supervision and able to prioritize assignments. Ø Ability to take verbal instruction and complete assigned project by established deadline; Ø Ability to be flexible in order to perform at the highest level as a team player. Ø Ability to take directions from supervisor and/or designee. Ø Ability to maintain confidentiality of all KIC operations and staff. Physical Activities Include Must be able to hear. Specific vision abilities include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to use hands to finger, handle, or operate objects tools, or controls; reach with hands and arms; climb or balance; stoop, kneel, crouch; and taste or smell. Each of the listed activities also requires standing, sitting, walking, bending, and stooping, climbing on stools and ladders, and routinely lifting and carrying items and boxes up to forty (40) pounds. Writing by hand and using a personal computer. Long hours in front of computer screen. Traveling occasionally, using air and/or water transportation, to out-of-town meetings and functions. WORKING ENVIRONMENT The environment involves the usual risks and stress of an office environment within an ambulatory health care facility. The employee may be exposed to communicable diseases. Safety precautions for employees such as blood drawing for screening of various diseases such as hepatitis, required immunizations, etc. are necessary. License(s) Possess current valid State of Alaska driver's license with a record acceptable to KIC’s Insurance carrier. Education High school diploma or equivalent required. Experience Progressive responsible administrative work that indicates ability to acquire the particular knowledge and skills needed to perform job duties. Prefer past experience/knowledge of Indian Health Service and Contract Health Systems. Motivation for professional and career advancement preferred.
