The Physical Therapist is responsible for performing studentevaluations, as well as providing and documenting therapy services inaccordance with plan of care developed for each student and per physician’sorders when applicable. Essential Duties and Responsibilities:
Performs student reassessments and regularly reevaluatesstudent for rehabilitation needs, and works in collaboration with the physicianand other members of the healthcare team to modify the plan of care asappropriate
Participates in developing the plan of care in collaborationwith the physician and other members of the health care team
When applicable, completes initial comprehensive assessmentof student and family to determine physical needs; provides a complete physicalassessment and history of current and previous illnesses
Assesses and evaluates and/or assists the physician inassessing and evaluating therapeutic, rehabilitative, function status (asapplicable)
Assesses muscle strength, mobility, gait, and Range ofMotion (ROM)
On incisions resulting from orthopedic surgery, removesstaples or sutures and manages incision line
Provides physical therapy treatment and establishes physicaltherapy plan of care
Instructs student, families, and/or caregivers in the use ofand care of therapeutic appliances
Determines priority needs for physical therapy
Reports student’s reaction(s) to treatment or changes incondition to physician and/or Building Nurse and Amergis Clinical designee
Initiates physical therapy program and instructs, consults,and/or advises other personnel and/or family/caregiver regarding certain phasesof physical therapy with which they may work with the patient, and explains thegoals of the physical therapy program by participating in case conferences
Trains student in use of prosthetic device(s) and use ofother assistive devices (e.g., walker); ensures equipment used in the course ofstudent treatment is in good working order
Uses appropriate equipment to meet student goals, cleans andmaintains equipment according to manufactures guidelines
Prepares and submits clinical and progress summaries basedon the attainment and progress of goals, and responses to treatment or changesin condition
Participates in peer consultation process
Supervises Physical Therapy Assistants according to thecompany’s policies/procedures and State regulations
Participates in quarterly medical record reviews asapplicable
Participates in care coordination with the company and otherpersonnel caring for the student to ensure that their efforts effectivelycomplement each other and support the objectives outlined in the plan ofcare
Provides/submits accurate and timely documentation inaccordance with the plan of care and company policy and procedures
Complies with applicable local, state, and federal laws andregulations regarding professional standards and practice standards
Minimum Requirements:
Current licensure as a Physical Therapist in the state ofpractice
Graduate of an accredited school of Physical Therapy
One (1) year of prior professional Physical Therapyexperience preferred
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB Questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
