In this position you will be responsible for working in conjunction with hospital case management/continuing care personnel to plan, coordinate and facilitate discharge planning for home care services for patients receiving respiratory and DME services.
Shift:
Monday - Friday: 8AM to 5PM
Job Requirements:
Applicants should possess a high school diploma or equivalent. Experience in the medical field and a strong customer service background is preferred, but not required.
