Transition In Place (TIP) Coordinator

The position of Transition In Place (TIP) Coordinator reports directly to the Clinical Director. The TIP Coordinator is responsible for the delivery of comprehensive clinical-based case management services to homeless veterans. The main function of this program is to assist homeless veterans in transitioning to permanent housing.

Responsibilities:

• Manages TIP program

Provide Case Management to Veteran Clients

• Reviews client files for quality assurance, including discharged charts

• Acts as a conduit between U.S.VETS - Prescott and the Veterans Administration and other community partners and programs to collaborate for service-delivery for veterans

• Assists with new client intakes, evaluations, and placements of veterans who are in need of services

• Acts as a liaison between CM staff and operations staff in making requests for supportive services

• Responsible for reporting client information to Clinical Director on various aspects of the program

• Audits all files a minimum of quarterly according to U.S.VETS standards

• Monitors program fee (rent) collection

• Participates in client's transition meetings

• Responsible for training new Case Managers in all aspects of CM responsibilities

• Collaborates with the Clinical Director and VA for entry/re-entry of clients

• Performs outreach to veterans and provides case management services as needed

• Performs crisis intervention, staffings, and home visits to address client needs and issues

• Develops and maintains collaborative partnerships with community agencies

• Responsible for data entry and management of program databases to compile demographics, track services, and generate reports

• Establishes scattered-site housing units in the community

• Responsible for fiscal management of supportive service funds

• Facilitates weekly program classes, as needed

• Facilitates a monthly town hall/community meeting with all veterans in the program

• Maintain overall organizational integrity within the program and compliance with all federal and other regulations

• Supervises data entry and updating of the Homeless Management Information Systems and responsible for accuracy of data

• Available for on-call emergencies

• Performs other duties as required

Requirements:

• Bachelor's Degree in social services field or other related field required.

• Minimum of two years of supervisory experience in related field. Two years of employment at U.S.VETS may substitute for supervisory experience.

• Experience working with homeless and/or veteran population preferred.

• Ability to work effectively with a diverse group of clients, staff, and community members.

• Excellent written and oral communication skills.

• Leadership and conflict management skills.

• Demonstration of personal and financial integrity in the workplace.

• Ability to take direction, work independently with minimal oversight, and to work within a team.

• Computer proficient in Microsoft Office and Internet.

• Valid driver's license required. Must meet company insurance requirements and complete a provided driver training course.
