POSITION: Habilitation Program Manager

DESCRIPTION: Habilitation is a service designed to teach individuals with disabilities daily living, self-help, and safety skills so that they may lead more independent, self-directed lives. Habilitation is provided in the client's home and/or out in the community. A Habilitation Program Manager designs and implements Habilitation programs for a caseload of clients receiving these services. Typical duties of a Habilitation Program Manger include:

1. Working M-F 8a-5p as well as occasional hours during the evening and weekend

2. Performing an on-call rotation of two consecutive weeks at least once every 10 weeks (two weeks on, 10 weeks off.) Duties include answering and managing emergency phone calls from 5p-8a M-F and all day Sat and Sun. Duties may also include working direct care shifts. On-call rotations last for two consecutive weeks.

3. Maintaining and growing a caseload

4. Scheduling shifts for clients and direct care staff

5. Writing objectives for clients and designing teaching programs for direct care staff to follow when teaching clients the skills necessary to achieve those objectives

6. Orienting direct care staff with clients and training that staff how to teach certain skills to that client

7. Monitoring direct care staff and their implementation of Habilitation programs

8. Holding staff meetings as needed and attending weekly program department meetings

9. Conducting initial intake of new clients

10. Meeting with clients/families minimally on a semi-annual basis

11. Attending meetings with case managers from other agencies as needed

12. Ensuring that all paperwork in client files is updated regularly

13. Documenting all significant conversations and/or correspondences with staff and clients

14. Reviewing data sheets and submitting them to the proper agencies

15. Maintaining client and agency confidentiality at all times

QUALIFICATIONS:

1. 18 years of age or older

2. Ability to obtain a current CPR certification

3. Ability to obtain a current First Aid certification

4. Ability to obtain an Arizona Level One Fingerprint Clearance Card

5. Ability to clear a DES background check

6. Ability to obtain TB clearance

7. A bachelor's degree from an accredited college or university in Special Education or a related field. One year of direct care experience involving teaching services may substitute for a degree.

8. Two additional years of experience working with children and/or adults who have developmental disabilities. Experience with individuals with autism highly preferred.

9. The ability to work in a team environment

10. The ability to handle difficult situations with clients/families

11. The ability to work a flexible schedule

12. The ability to learn and operate UCPSA program software

13. The ability to work independently

14. Valid driver's license and reliable transportation

15. The ability to lift and transfer clients (up to 50 lbs)

16. Bilingual in English and Spanish preferred

17. Applied Behavior Analysis experience preferred
