RN Care Coordinator

Children’s Clinics is seeking a Part-Time RN Care Coordinator. As a member of the Clinical Services team, the RN Care Coordinator will join a strong transdisciplinary team serving a diverse patient population consisting primarily of children with complex medical needs including cerebral palsy, spina bifida, neurogenic/neurologic disorders, autism spectrum disorder, and Down syndrome. The RN Care Coordinator will directly impact patient outcomes: complete basic patient assessments; act as a liaison between patients, families, and physicians; manage medication changes and prescriptions refills; interface with various health plans; triage calls. QualificationsSUMMARY This position is responsible for planning, directing, providing and documenting nursing care to patients of the Children’s Clinics. ESSENTIAL FUNCTIONS

Adheres to approved standards of triage and nursing practice throughout the assessment, planning, implementation and evaluation phases of the nursing process.

Facilitates scheduling of patient appointments to ensure urgent patient needs are met . Assesses the severity and urgency of telephone calls and triages patient calls and develops a plan using the triage process and nursing assessment.

Reviews the medical record and coordinates the acquisition of patient records, procedure reports, diagnostic test results, data base information, slides and any missing reports or information. Assesses and documents patient intake information and determines nursing actions required.

Provides supervision/guidance to Medical Assistants to assure effective patient flow as needed.

Assesses, plans, implements and evaluates the nursing care of patients to include nursing assessments; observing, recording and reporting pertinent patient status information; administering medications and vaccines as well as other nursing procedures; and the provision of patient/family education.

Collaborates with physician and other care members in the assessment of the patient’s status and delivery of patient care. Participates in discharge planning and performs necessary patient/family teaching based on communication with the physician and other health care providers. Provides medically necessary patient information to agencies/organizations.

KNOWLEDGE, SKILLS AND ABILITIES

Skill in various software applications. Basic computer knowledge. Competent using an Electronic Health Record system.

Skill in intervention and collaboration

Skill in problem solving, critical thinking, and detailed organization

Ability to communicate effectively, both verbally and in writing with patients, their families, clinic staff and community members.

Ability to transcribe and execute orders for laboratory studies, x-rays, special test, medications, treatments, and other matters pertaining to patient care.

Ability to operate personal computer and related office equipment as needed to perform job function.

MINIMUM QUALIFICATIONS

Education:

Associate’s Degree in Nursing from an accredited nursing institution required

Bachelor’s Degree in Nursing preferred

Experience :

Two or more years pediatric nursing experience preferred

Bilingual English/Spanish preferred

Licenses and Certificates :

Registered Nurse License in the state of Arizona required

BLS certification required

FINGERPRINT CLEARANCE

Employees are required to maintain a current Department of Public Safety Level 1 Fingerprint Clearance Card. Proof of a current fingerprint clearance card must be received within 90 days of employment.

The noise level in the work environment is usually moderate.
