The In-Home Outreach Counselor assumes the following duties and responsibilities:
Provide in-home outreach counseling to provide support and educate families on
parenting skills, positive discipline, laws regarding child abuse and neglect, goal setting,
and family organization;
Provide supplemental services: child focused activities; child follow-up visits; parenting
training; teaching and demonstrating homemaking skills; counseling on substance abuse,
domestic violence or anger management, and career need issues; substitute adult role
model; transportation; and emergency housing;
Develop and utilize community resources to provide linkage services for drug testing,
substance abuse assessment and treatment services, etc.
Complete intakes and assessment forms and maintain up-to-date FAF primary web-based
Electronic Case Records system and hard copies case records and back up documents on
each client and family served, and other reports in a timely manner;
Be available to provide emergency services as needed;
Attend MCPC and all other meetings and trainings required by the program;
Strictly adhere to confidentiality, privacy and Cyber security policy and protocol;
Performed other related duties as assigned.
Qualifications:
Bachelor’s Degree in Social Work, Psychology, Marriage and Family Counseling, or a
closely related field, and have a minimum of fifteen months of experience providing
direct client services in a social service setting. Master Degree in social work or related
field is preferred.
Bi-lingual/bi-cultural in English/Cambodian; English/Spanish; English as an African
American Cultural Broker;
Strong administrative, written, and oral skills;
Proof of legal residency and legal right to work in the U.S.; and
A valid California driver's license and proof of insurance
Salary: $53,000 - $75,000 Annually plus Excell ent Fringe Benefits package.
