Mentor's work closely with individual with Disability. By providing program approved activities that promote community inclusion. The program encourages persons receiving services with disabilities to become vital parts of society. It also, encourage individual to make decisions as to what activities they want to participate in and open the doors to explore different activity. We help to advocate for individual choice making and empowering the individuals to make informed decisions about their own life experiences. Promote healthy interactions in the community with community members.

• Ensure that Individuals get out into the community by taking them to activities of their choice and helping client obtain their goals.

• Follow the approved calendar of appropriate community activities.

• Responsible for 1 to 4 individuals during program.

• Ensure that everyone stays within the group.

• Maintain accountability of every individuals that's in your care.

• Ensure the safety and well-being of all clients and ensure that everyone is treated fairly with

• dignity and respect.

• Ongoing communications with supervisors, team members, family members regarding any issues, concerns, problems (i.e. programmatically and medically).

• All duties are always to be carried out with the utmost professionalism and appropriateness.

• Observing confidentiality of clients and company operations.

• Provide transportation door-to-door for each PRS.

• Must attend and show competency in all company required training.

• Be responsible for daily log notes.

• Must be able to display good judgment and common sense.

Must have a High School Diploma or GED

Must be 21 years or older

Must be able to pass criminal background test

Must be able to pass a drug and alcohol test

Must have a valid driver's license, insurance and reliable transportation.

Must be available to work Monday-Friday 8:00 am- 5 pm and some weekends on rotation.

Able to work up to 40 hours a week.

Must have experience working with individual with disabilities or willingness to be trained.
