Job Description

Duties and Responsibilities:

• Performs resident assessments as it relates to new admission, change of condition and periodic assessments.
• Coordinates admission, discharge and transfer of residents.
• Collaborates with physicians and members of multidisciplinary team.
• Contributes to the development of resident care plans.
• Administration and documentation of medications and treatments per physicians order
• Evaluates resident responses to nursing interventions.
• Provides emotional, psychological and spiritual support to residents and family.
• Responds to resident and family concerns and seeks ways to resolve issues with multidisciplinary team.
• Provides health education to residents and family members
• Maintains safe and clean working environment by following policy and procedure.
• Adheres to infection control policies, medication administration and storage procedures and controlled substance regulations.
• Orders and receives medication delivery from pharmacy.
• Supervises nursing care provided by CNA's and LPN's.
• Performs other duties or functions as directed.

Qualifications

• AD or BSN from accredited nursing program
• Current and active Colorado nursing license.
• Current BLS certification.
• Clinical skills.
• Ability to work as part of a team.
• Multitasking, listening and verbal communication.
• Organizational skills.
• Experience in skilled nursing, long term care and/or geriatrics is a plus.
• Able to pass drug screen and criminal background check.
