Job Description Summary:Job Description:As a Pediatric Primary Care Provider for Community Health Center, you will:

Provide a full scope of medical services largely geared to underserved populations

Receive strong clinical support services and work in an innovative practice environment utilizing Advanced Access scheduling

Be part of a multi-disciplinary practice that offers a blend of individual and population based care

Physician Qualifications:

MD/DO from approved medical/osteopathy school and graduation from accredited residency program in pediatrics

Must be board-certified within 24 months of commencing employment at CHC and hold license or be eligible for license in State of CT

Bi-lingual skills English/Spanish preferred

We do accept NP's for this position

APRN Qualifications:

Graduate of an accredited Master of Science in Nursing program with a specialty as a Pediatric Nurse Practitioner ANA certification as a Pediatric Nurse Practitioner

State of Connecticut APRN and RN licensure (within 12 months) required.

Bi-Lingual Skills English/Spanish preferred

Certifications Required: Medical - State of Connecticut Licensure

Physician Assistant Qualifications:

Graduate of an accredited Master program in the study of physician assistant.

Maintains appropriate board certification, licensure, and professional organizations.

The majority of our patients have health coverage under Medicaid. APC provides opportunities for disease prevention and health promotion as well as early detection of problems. APC guides patients through the health system, including appropriate referrals for services to other health professionals. Job Summary: The Patient Service Coordinator is responsible for providing outstanding and professional customer service to ensure operational efficiency and promote a positive patient experience. Job Responsibilities: · Greets and registers arriving patients, completes registration forms and verifies insurance, including scanning and faxing forms as necessary. · Answers incoming calls, evaluates priority, and directs calls according to the urgency and subject matter. · Mails information to new patients prior to visit in accordance with department policies and enters / updates patient demographics. · Schedules new patient appointments and informs patients of essential preparation requirements prior to visit per department policies (referrals, x-rays, medical reports, etc.). · Ensures that office appointment schedule is coordinated with clinical staff activities. · Reviews assigned appointment confirmations to ensure accurate schedule and appropriate communications with patients; completes appointment confirmation activities according to department policies. · Schedules follow-up appointments. Verifies insurance coverage and obtains authorizations. · Orders department supplies. · Maintains schedule for conference room. · Maintains a clean waiting room and office area. · Defines and resolves patient/physician issues/problems, bringing concerns to team lead or manager as necessary. · Completes facility billing according to facility level criteria.
