Position OverviewThe Peer Navigator supports the Employment Transition Model (ETM) Grant by helping youth and young adults (ages 16–24) with disabilities to prepare for and succeed in employment and training opportunities. This role focuses on peer mentorship, job readiness, and resource navigation to remove barriers to work and education. Peer Navigators are young adults with lived experience who want to pursue a career in Human Services while helping others reach their potential.Key ResponsibilitiesProvide one-on-one and group of peer support to youth participants in the ETM program.Assist with developing individualized career or employment plans.Help participants access community resources (transportation, housing, support services).Enter participant progress and case notes into CT Hires / DOL Case Management System.Collaborate with Workforce Counselors, ADS/BRS Case Managers, and employers.Support youth in completing training, job readiness, and employment goals.Participate in leadership meetings, professional development, and ETM grant training.Assist Young Adult Program Case Managers in conducting Young Adult Orientations and communicating the value of the programCommunicate with youth, community programs via phone calls, texts and emails  Other duties as assigned
