Position Overview
We are seeking Registered Nurse (RN) Case Managers to join our dedicated healthcare team. In this role, you will oversee and coordinate comprehensive care plans for clients, providing 1:1 care in their homes. You’ll enjoy flexible scheduling with weekends off, allowing for a balanced professional and personal life.
Responsibilities
As an RN Case Manager, you will:
Implement, evaluate, and supervise client care plans.
Provide total client care, including:
Nursing assessments
Organizing and managing caseloads
Coordinating care and medical records
Supervising field staff
Administering medications and treatments per physician orders
Monitoring physical and psychological health
Educating clients and families on care needs
Assess and respond to symptoms, initiate remedial actions within legal scope, and report significant changes in condition.
Make sound clinical judgments and maintain thorough documentation.
Qualifications
Current RN license in the state of Connecticut
Strong clinical judgment and independent decision-making skills
Excellent written, verbal, and interpersonal communication skills
Ability to prioritize tasks and manage multiple assignments effectively
