Working closely with the population health teams, primary care providers (PCP’s), networked facility partners, and practice operations at Privia Health, the Health Advisor will provide care coordination and support for Privia’s at-risk patients. They will provide inbound and outbound telephonic support for care management team. Primary work will include outbound calls to patients to provide coordination services, fill gaps in care, and promote improvements to patient outcomes. Assist physician practices in identification and completion of quality care goals through direct patient outreachCollaborate effectively with integrated care team to support care coordination for patientsReview patient non-compliance lists and conduct telephonic outreach to ensure timely disease specific and wellness visitsAnswer and resolve business office inbound calls related to population health, care coordination, or other basic clinical activitiesMotivate patients to be proactive about their healthcareAssist patients in accessing care at the appropriate sourceMake appropriate referrals to internal care team, community resources, and disease management programsDevelop relationships with the care coordination teamsCoordinate home care referralsAssist in scheduling PCP or specialist appointmentsRoute clinical calls and referrals to the appropriate resource: Nurse care manager, pharmacist, social worker, or appropriate resourceMust comply with all HIPAA rules and regulationsOther duties as assigned
