Summary:
The Developmental Disability Waiver RN will oversee the services and support provided to individuals, ensuring that the performance of assigned aides/attendants meets established standards through regular supervisory visits. He/she will ensure compliance with the Individual Service Plan (ISP) and adherence to relevant regulations set forth by the Virginia Department of Health, the Department of Medical Assistance Services, the Department of Behavioral Health and Developmental Services, and other applicable federal and state regulatory agencies.
Key Responsibilities:
Perform supervisory visits within the required timeframe.
Review the ISP and make changes as appropriate based on the individual's needs.
Evaluate the effectiveness of home care services by obtaining feedback from the individual and/or the primary caregiver (PCG), and the assigned aide/attendant.
Coordinate services with the Care Specialists and DBHDS support coordinator.
Submit assessments and other required documentation promptly.
Participate in interdisciplinary team meetings as scheduled.
Work closely with the individual, family, and support coordinators regarding planned or ongoing services.
Discuss with the Nurse Manager and/or the Director of Nursing any problems concerning the individual or the services and support being provided to the individual and develop appropriate and effective solutions.
Assist with or perform referral intake, admission, resumption of care, discharge planning, and other assessment activities.
Participate in office meetings and staff development training.
Assist with performance reviews of the aide/attendant and ensure their competency.
Collaborate with other departments based on specific projects assigned.
Adhere to federal, state, and accreditation requirements.
Maintain licensures and/or relevant certification as required.

Minimum Job Requirements:
Active, unencumbered RN license in Virginia or multi-compact state
At least one (1) year experience working as a nurse in a clinical or public health setting
Active professional liability insurance with appropriate coverage
Valid, unexpired driver’s license
Authorization to work in the United States
Negative PPD test or negative chest X-Ray if PPD was positive
Active Basic Life Support Certificate or higher
Other Important Job Requirements:
Good command of the English language, written and spoken (bilingual a plus!)
Proficiency in Microsoft 365, Google apps, and Adobe Acrobat PDF
Experience with electronic health record system
Ability to prioritize tasks with considerable efficiency
Excellent customer service skills
