As a part-time Nurse Care Manager (NCM) at Paladin Life Care, you will be a trusted advisor to clients and their families, conducting assessments and developing comprehensive Plans of Care. Your focus will be on identifying medical and functional healthcare needs, living environment, and emotional stability. You will work collaboratively with the Client Care Manager to ensure appropriate resources are available to support our clients' needs.

Qualifications:
Licensed RN or LPN in Virginia; Multi-state licenses to include Maryland and D.C. are preferred.
5+ years of Clinical Experience. Experience in Care Management, Life Care Planning, Psych, Medicare Set-Aside, or Legal Nursing is beneficial but not required.
Must be Able to Travel within the Metro DC Area to visit clients and attend appointments.
