Position Overview:
We are currently seeking a dynamic and compassionate individual to join our team as a PRP Counselor / Case Manager. In this role, you will provide psychiatric rehabilitation services and case management support to clients, guiding them towards recovery and independence.
Responsibilities:
- Conduct comprehensive assessments and develop individualized treatment plans for clients
- Provide individual and group counseling sessions to address mental health needs and facilitate skill-building activities
- Coordinate with interdisciplinary team members to ensure holistic care and support client goals
- Assist clients in accessing community resources and support services, including housing, employment, and healthcare
- Advocate for clients' rights and needs within the community and healthcare system
- Monitor progress and document client interactions and outcomes in accordance with organizational policies and procedures
Qualifications:
- High School Diploma
- (Bachelor's or Master's degree in Counseling, Psychology, Social Work, or related field) a plus
- (Previous experience working in mental health or rehabilitation settings preferred) a plus
- Strong interpersonal and communication skills
- Ability to work effectively in a team-based environment
- Knowledge of psychiatric rehabilitation principles and practices
- Certification in CPRP (Certified Psychiatric Rehabilitation Practitioner) a plus
- Experience in case management or social work is desirable
Salary:
- Pay range: $25 to $40 per hour, commensurate with experience and qualifications.
